NSA and its clients are looking for the best service centers available.  If you are interested in learning more about the requirements to become a member of the most progressive network in the country, please follow these easy steps:

  1. Download the Service Provider Profile by clicking on the link below.
  2. Download the Service Provider Agreement by clicking on the link below.
  3. Review the requirements and the Service Provider Agreement.
  4. Complete the first page of the Service Provider Agreement.
  5. Sign the signature page of the Service Provider Agreement.
  6. Complete the Service Provider Profile.
  7. Scan both documents & e-mail them to virginia@nationalservicealliance.com 
  8. You can also fax them to 801-748-4188.
  9. Add National Service Alliance to your general liability insurance policy as an additional insured and ask your insurance company to send a certificate to National Service Alliance, 6762 South 1300 East, Salt Lake City, UT 84121   

Note on Background Checks

Some NSA clients now require service providers to have a background check performed for each technician that performs service in a customer’s home.  The background includes:

  • A nationwide criminal history database search with county level follow-up searches for any records found.
  • A county level criminal history search for each county of residence for the last seven years.
  • A statewide sex offender registry search as allowable by law. 
  • Drivers licence check.
